I understand that my utility meter will be read on a quarterly basis and that I will receive a bill from the City for utility services. I acknowledge that this bill will include base charges and any charges for actual usage recorded during the billing period.

I understand that if I choose NOT to provide a Customer Temporary Address to the City of Hastings (as requested above) my City utility bill will be sent to my Hastings address and will not be forwarded by the post office.

I understand that failure to receive my utility bill at a forwarding address may result in late fees being applied to my utility account.

I understand that this arrangement will remain in effect until I contact the City of Hastings to report any changes, such as updates to my temporary address or service dates.
